How It Works:
Complete the membership form here. Membership starts on the 1st of the month following submission.
On the 1st of the month, you will be sent an invoice by text and email to store a CC, FSA or HSA.
Following payment, your name will be flagged as a “member”, and you will be invited to create a patient portal. In this portal, you can schedule consults for Functional Medicine, Hormone Health, peptides, weight loss.
You can order NAD+ here. Make sure to mark yourself as a member.
For IV infusions & injections, schedule or present to Vital Infusions per normal. When you arrive, notify our nursing team that you are a member. They will provide you with a card to track your monthly injections and infusions.
